We are happy to hear it! Scheduling an intake is easy. We just need a little information from you first.
- Fill out an Intake Form: click on “Become a Client” and complete the form.
- An Intake Team Member will reach out: Once the form is submitted, a member or our intake team will reach out to you, or to the designated parent/guardian for children under 18.
- Complete your intake: Our intake team member contact you to complete any needed information, verify insurance, find the right provider for you, and schedule an initial appointment.

Welcome back! If you’ve seen us in the last year, you can contact our front desk to schedule, cancel or change appointments on (781) 429 7755 or email [email protected]. Alternatively, you can easily log on and do this through our client portal.
If it’s been more than a year since we saw you, please complete a new intake form based on the instructions above.
